If you are a new patient, please sign in to the Patient Portal before your first visit to complete your health history. Use the username and password mailed to you with your appointment confirmation to sign in.

If you have questions about the patient portal or your username and password, please call us at 210-648-2273. Completing your patient history forms takes about 30 minutes. If you must sign off before completing forms, return to the portal at your convenience and pick up where you left off.

INSURANCE ID
Please bring your insurance card or Medicaid identification form to each appointment.

MEDICAID PROGRAMS
In Texas, several Medicaid programs are available. To avoid problems with your account, please notify us immediately if you change from one Medicaid program to another.

PAYMENT
Payment for medical services is required at the time of your appointment.

FOR PATIENT SAFETY - CHILDREN NOT PERMITTED
Children are not permitted in the office. We thank you for your cooperation and understanding. Childhood diseases endanger the health of pregnant women and their unborn children. For the well being of all our patients, please make arrangements for child care during your office visit.

HOUSE RULES
Help us keep our reception area clean. Please no food or beverages.

APPOINTMENTS
If you are more than 15 minutes late for your appointment, we may have to reschedule you. If you cannot keep your appointment, please call the office at least 24 hours in advance to reschedule.
